Basic Question Types

The Form Builder supports all the traditional question types you’d expect, including:

  • Short Answer – for brief text responses

  • Paragraph – for longer, open-ended responses

  • Number – for numeric inputs

  • Checkboxes – for multiple selections

  • Dropdown – for single-choice answers

  • Multiple Choice – for predefined single selections

  • Multiple Choice Grid – useful whenever you want to map a pair of attributes to a single choice. (one option at the intersection of two variables)

  • Star Rating – for satisfaction or rating scales

  • Linear Scale – for numerical scales (e.g. eNPS)

  • The Info Box is a non-interactive question type that allows organizations to:

    Display custom messages, guidelines, or reminders directly inside the form.

    Guide users through the questionnaire without relying on external tools or documents.

    This is ideal for embedding short instructions, policies, or helpful tips within a form.

Q7Leader Question Types

In addition to the standard question types, the Template Builder includes Q7Leader Question Types, which let you integrate:

Teamscan questions

Q7 dimensions you can find in the TeamScan and SelfScan:

  • Roles & Responsibilities
  • Competence
  • Performance
  • Talent
  • Vision & Values

They work just like regular question types, helping align conversations with your organization’s performance and development frameworks

These fields are not prefilled with any content from the Teamscan. It’s entirely up to the manager to complete them as they see fit. Ideally, they will be transparent, but they can choose exactly what they want to share with their team members.

RDP

Our builder includes the ability to directly add a member’s RDP to the template.

This feature allows both the Manager and the Team Member to review the RDP without leaving the conversation, ensuring context, continuity, and time efficiency.

The RDP is not editable by the conversation participants. If HR wants them to communicate any changes that should be made to the RDP, we recommend using it together with a paragraph field where participants can provide their feedback. HR can then decide if and how to incorporate it.

Objectives


The Objectives question type allows organizations to embed and manage objectives directly within the conversation form.

Key features include:

  • Direct access to objectives without leaving the page.

  • The ability for users to view, edit, or update objectives within the conversation flow.

This question type helps align individual conversations with organizational goals and ongoing performance tracking.

You can configure the objective question type in the following way:

Filter objectives by cycle and type.

This allows to organize objectives into separate sections such as Company, Business, and PDP.

Create multiple sections using the same question type.

For example:

  • Section 1: Company objectives
  • Section 2: Business objectives
  • Section 3: PDP objectives

Apply custom limits to each section.

You can set:

  • An exact weight that the objectives in that section must add up to.
  • An exact number of objectives that must be created.

Understand the validation rules.

Both weight and count are exact requirements, not minimums or maximums.

  • Submission is blocked if the total weight does not match the requirement exactly.
  • Submission is blocked if the number of objectives does not match the exact count defined in the template.

By setting these parameters, HR can ensure that each objective type is completed in a structured and consistent way before the conversation can be submitted.

Protected Paragraph


The Protected Paragraph is an enhanced open-ended question designed for controlled editing rights. When creating this question type, form creators can define who can respond:

  • Manager only, or

  • Member only

This ensures sensitive or managerial comments stay secure, while still promoting structured dialogue.

Logic Questions Types

Flow control enables you to design questions that follow logical rules, allowing your form or survey to adapt intelligently based on user input.

Depending on the user’s answer, the form can automatically redirect to another section or skip certain questions.

For example:

  • If the user answers “Yes”, they can be directed to an additional section of questions to provide more details.

  • If they answer “No”, the form can move directly to the next question, skipping irrelevant questions.
